HEALTH INSURANCE INFORMATION

Health insurance is required for all full time students at CRCDS (those enrolled for a minimum of 9 credit hours/semester).
       
Students may purchase a basic student health plan through CRCDS for $885 for the 2011-2012 year.  However, this plan is only a blanket accident and health plan.  Please click here to download the summary of current benefits.
               
Students are also encouraged to explore other low cost health insurance options offered by New York State and local insurance companies so that they may make an informed decision based on their individual health care needs.  More information about health insurance options may be found at the following links.
                          
If you have comparable insurance and are not in need of the school sponsored plan, the waiver form must be completed online at this link or you will be billed for the school sponsored plan. The fall deadline to opt out of the plan is 9/13/11. For students beginning studies in the spring, the deadline is 2/14/12.
                                     
Part-time Students and Dependents

Part time students and dependents of students may also voluntarily enroll in the school-sponsored plan at the following 2011-2012 rates:

Student:   $885
Spouse:    $1,316
Child(ren): $1,316
Spouse and Children: $2,632

Students enrolled in the plan may also visit the Niagara National website for full information about the CRCDS Health Insurance plan, to print an ID card, to obtain claim forms, or to view other optional insurance products.
